It’s simple to transfer your prescription. All we need is where you currently fill your prescriptions, your insurance information as well as your phone number.
We accept a majority of prescription insurance plans. You are responsible for any co-payment required by your insurance carrier.
Kubat retail pharmacy delivers prescriptions in the Omaha, Fremont and surrounding areas. If you get your prescription called into us by 10am we’ll have it delivered the same day. If it was called in after 10am we’ll get it to you the next day. We do not currently deliver on the weekends.

Compounding is the art and science of preparing custom medications. At times, an individual may not be able to use manufactured medications. Maybe a dosage form is not commercially available, or the individual may be allergic/sensitive to dyes, preservatives or additives. Compounders can custom make medications in the best dosage form for the individual.
We are located at 4924 Center Street Omaha, NE 68106
The compounding fax number is 402-561-1252.
As a general policy, we ask for 24-48 hours to complete your custom medication.
Since the medication is custom made for the individual, each compounded medication has varying preparation times depending on a variety of factors such as:
The time it takes for each medication can be discussed with one of our compounding team members. Some medications can be ready the same day and others may take a day or longer to complete.
Yes we can ship your medication to you. The cost to ship to anywhere in the state of Nebraska and Iowa is free. Other states there is a $5 per shipment fee. If your medication requires overnight shipping please contact our lab for accurate pricing (402) 558-2474. We take precaution to pack your medication in temperature sensitive packaging to protect it from outside elements and may have to send it by expedited shipping.
Our compounding lab is open Monday – Friday 9am to 5:30pm.
We have an automated system that will contact you when your prescription is ready.
You can pick up your prescription on the weekend. Our retail pharmacy will be able to get your prescription if you were notified it’s ready for pick up. However, our compounding pharmacy is closed and a compounding pharmacist may not be present to answer questions regarding your compounded medication. You can reach our compounding team Monday – Friday 9am to 5:30pm at 402-558-2474. Since our compounding pharmacy is closed on the weekend we won’t be able to fill any medications over the weekend.
We are two separate pharmacies in one building. Sometimes this creates confusion because our retail pharmacy has extended hours. You can pick up in the same place, but if you are picking up for both pharmacies they may be stored separately. Kubat Custom Healthcare is a compounding only pharmacy while Kubat pharmacy fills manufactured products.
Some insurance plans cover compounded medications. You may want to check with your insurance company for your specific coverage. Kubat Custom Compounding does not bill insurances directly at this time. We supply you with a Universal Insurance Claim Form that you may send to your insurance carrier, If they cover compounds, they will reimburse you directly minus your copay. Please note, we accept all major insurance plans in our retail pharmacy.
A compounding pharmacy makes your medication specifically for you. Other pharmacies that do not specialize in compounding do not have the equipment or chemicals to make custom medications.
Please give the compounding pharmacy a call at (402) 558-2474 and they’ll send you a universal claim form.
In many parts of the country, Competitive Bidding will require you to obtain certain medical equipment from specific, Medicare-contracted suppliers in order for Medicare to pay. Not all products are subject to competitive bidding in the same area.
Competitive Bidding areas are designated based on the zip code of your permanent residence on file with Social Security. To find out if your zip code is affected by Competitive Bidding, call 1-800-MEDICARE (1-800-4227). You may also visit Medicare.gov and lookup suppliers in your area by zip code (a notice will appear if your area is subject to Competitive Bidding).
Effective July 1, 2016, Kubat Pharmacy is pleased to announce we’re able to file claims to medicare for products covered by the Medicare DMEPOS Competitive Bidding Program. This program affects Medicare reimbursement in the following categories:
